Clinical reading of the story of the disease : From intertextuality to the merging of horizons. A case study

Résumé

How can we account for the inexpressible in the traumatic experience of the disaster? Based on a clinical situation encountered by a clinical psychologist in a resuscitation service, this article approaches the mental representation of organic disease in its textual and intertextual aspect, by paying attention to the weaving of the semantic relations which constitute any speech. The notion of intertextuality allows to specify the articulation of the psychic and its environment, that is both the personal history of the patient and the collective order of culture. Self-narrative employs discursive forms of narrative traditions of the “recordable” that emerge as models of “reading”. The story is addressed to the clinician and inscribes the narrativity in a horizon of expectation (H.-G. Gadamer) governed by the quest for an answer. For its part, the clinical reading rests on a different horizon of expectation, concerning what this narrative means. From the phenomenological point of view, the clinical moment of listening and restitution of the narrative’s intertextuality could be approached through the notion of “fusion of horizons”. The restitution of the intertextuality appears as a way between the prefiguration of the events told and the re-figuration of the history.
Comment rendre compte de l’indicible à l’œuvre dans l’expérience traumatique de la catastrophe ? À partir d’une situation clinique rencontrée par un psychologue clinicien dans un service de réanimation, le présent article aborde la représentation mentale de la maladie organique dans son aspect textuel et intertextuel, dans l’attention au tissage des relations sémantiques qui compose toute parole. Le récit de soi emprunte des formes discursives aux traditions narratives du « racontable » qui se présentent comme des modèles de « lecture ». La textualité qui se déploie à travers ce récit adressé au clinicien inscrit la narrativité dans un horizon d’attente gouverné par la quête d’une réponse. De son côté, la lecture clinique de ce texte reçu repose sur un horizon d’attente différent, concernant ce que signifie ce récit. Du point de vue phénoménologique, le moment clinique de l’écoute et de la restitution de l’intertextualité constitutive du récit pourrait alors être à penser à travers la notion de « fusion des horizons » (H.-G. Gadamer), qui porterait à la parole l’intelligibilité du texte dont le clinicien doit se faire l’herméneute. La notion d’intertextualité permet de préciser l’articulation du psychique et de son environnement, c’est-à-dire aussi bien l’histoire personnelle du patient que l’ordre collectif de la culture ; son analyse se présente comme une voie praticable entre la préfiguration des événements racontés et la re figuration de l’histoire.
