Why to organize the secondary planned management of a Strasberg E bile duct injury occurring during laparoscopic cholecystectomy
Résumé
Biliary tract injury during laparoscopic cholecystectomy is a serious complication that leads to multiple additional examinations, a longer length of stay, and a significant decrease in the quality of life. This unexpected complication shifts the programmed management into the rhythm of a surgical emergency and the patient from the ambulatory atmosphere to that of long-term hospitalization. We present a discussion of the interest of the secondary planned management of a Strasberg E bile duct injury, illustrated by a concrete clinical case. The strategy that we describe allows this emergency situation to be put back into a planned, programmed situation, under good clinical and biological conditions, after a complete workup, including discussion in a multidisciplinary consultation meeting and management by another operator who specializes in biliary pathology, while giving the patient a fair and optimal explanation.
