N°251 – Multivariate lesion-symptom mapping in stroke patients with motor deficits

Résumé

Objectives Focal structural damage to white matter tracts can cause functional deficits in stroke patients. In this study, we developed a multivariate lesion-symptom mapping method based on structural disconnection maps to examine brain structures associated with left/right motor deficits. Content Imaging data from more than 300 stroke patients (63.9 ± 10.5 years) were included in this study from the GRECogVASC cohort. Clinical and neuropsychological examination and MR imaging were performed six months after the stroke. Lesions were first manually segmented on 3D T1 MRI data and normalized into the MNI152 template. The conventional voxel-based lesion-symptom mapping (VLSM) method was used to identify lesioned voxels associated with left/right motor scores with p < 0.005 (1000 permutations) using binary lesion masks from patients. A probabilistic structural disconnection map was then generated for each patient using DSI-studio by using diffusion-weighted imaging data from 400 healthy aging controls selected from the CamCAN repository. The multivariate support vector regression analysis was used to obtain statistical maps using the bootstrap sampling method for the left and right motor deficits. Our results achieved high sensitivity in identifying brain structures including the corticospinal tract associated with motor deficits in comparison with VLSM, which resulted in high false positives, especially in the right hemisphere associated with the left motor deficit. The multivariate structural disconnection-symptom mapping outperformed the lesion-symptom mapping using binary lesion masks.
